RESPONSIBILITIES:
The Treatment Court Liaison/Certified Recovery Peer Advocate assists in the development and monitoring of the recovery systems of UConnectCare (formerly known as GCASA). QUALIFICATIONS:
Must have a minimum of a high school diploma or jurisdictionally certified high school equivalency. Must have or be willing to obtain within the first six months of employment the Certified Recovery Peer Advocate (CRPA). 

REPORTING RELATIONSHIP:

Case Manager - GBHI Grant

Case Manager

RESPONSIBILITIES:
The Case Manager will assist individuals and provide support for emergency, transitional, and ultimately permanent housing obtainment for people with substance use disorder (SUD) who are homeless. He/she will be responsible for coordinating substance use disorder (SUD), mental health, and physical health care provider communication and navigating health insurance and safety net program enrollment and maintenance processes for people with SUD who are homeless, at imminent risk of homelessness, and/or have histories of chronic housing instability. Minimum of 1 year of case management or similar experience required. 2 or more years and/or intensive case management experience preferred. Bachelor’s degree preferred.

Status: Full-time hourly non exempt based on a 35-hour week. This is a grant funded position.

RESPONSIBILITIES:
Carries client case load and ensures quality care to residents. Provides individual and group counseling services, as well as addresses family needs and referrals. Completes paperwork requirements in accordance of regulations of governing body and maintain quality, up-to-date clinical records while effectively utilizing agency electronic health record. Ability to demonstrate understanding of various counseling modalities and philosophies for the treatment of substance use disorders, as well as trauma informed systems of care.

REPORTING RELATIONSHIP:

RESPONSIBILITIES:
Full time nights, and part-time and full-time days. Weekends.  LPN is responsible for related services for each patient and appropriate linkage to internal/external services. The LPN will participate in the Interdisciplinary Team meetings when necessary to inform the clinical team about medical issues, under the direction of the Physician Assistant.

QUALIFICATIONS:
Must be a Licensed Practical Nurse in the State of New York. Must have a current CPR/First Aid certificate or obtain certification within 90 days of hire. Must display the use of appropriate independent judgment to determine the appropriate course of action in resolving problems and in emergencies. Registered Nurse (RN)

SUPERVISION: The Registered Nurse will report to the Nurse Manager  

DUTIES:
Per diem, part-time and full-time positions available. Evenings, weekends and holiday coverage needed. The nurse is responsible for all nursing related services for each patient and for linking patients to the physicians and to outside medical resources. They participate in the Interdisciplinary Team Meetings when necessary to inform the clinical team about medical issues and is available for emergency medical situations. The nurse provides all services under the supervision of the Director of Nursing. The nurse is responsible to provide medical and history screens, providing routine nursing care and overseeing the medical and psychiatric management of the patient in coordination with the attending physician. They are also a member of the Treatment Team and will provide input into the development of the Master Treatment Plan.
